Question: I have found your site very informative and a lot of questions have been answered. I am happy to know that I am not the only ignorant person when it comes to their own vaginas. I didn't see one question though, on my vagina it seems as though I am falling out, if that makes sense. I thought that I used to just have the opening into my vagina, but now after three kids (the youngest 10 months) I think I look like the inside is protruding. Is this normal, what is it, and can I fix or change it?

Answer: Some amount of change is to be expected following multiple births, since the tissues must stretch to allow the passage of a baby's head. I suspect part of the problem could be loss of pelvic muscle tone. The pelvic muscles help hold things in place. Learning and doing Kegel exercises may help with the muscle tone and help return your vulva to its prior appearance, keeping in mind loose folds of tissue are common in this area. Kegels will also increase your sexual awareness and pleasure doing intercourse and orgasm. Kegels are described on the pages about Virginity.

It is possible you have experienced a partial prolapse of the uterus, so you should be examined by a doctor to rule this out. This happens when the ligaments that hold the uterus in place become injured or stretched causing the uterus to no longer be held in proper position within the pelvic region. This may require surgery to repair.

If the doctor performed an episiotomy to widen the vaginal opening during delivery, this too could cause the appearance of the vulva to change, depending on how large an incision was made and how well they lined the layers of tissue up when suturing everything back together. Multiple episotomies may result in increased scar tissue and disfigurement.

I would advise against cosmetic surgery, though some doctors may suggest it. Anytime surgery is done there is risk of injury to the nerve and blood supplies of the vulva and vagina. While your vulva may look nicer to you, it may not function as good as it once did, and could become the source of discomfort and pain. Kegels are a more natural way of getting your body back in shape following childbirth.
